Hardwiring Health Literacy into Health Information Systems

Farrah Schwartz is the Manager, Patient Education and Engagement at the University Health Network. In this role, Farrah promotes health literacy awareness and skills through staff education, program consultation and resource development. She also regularly works with health care providers to provide usable, reliable health information and help ensure their patient and family education is inclusive, engaging and health literate.
Farrah has been working in the field of patient and family education for over 15 years, in both the rehab and acute care hospital settings. She is one of the founders and the administrator of the Canadian Health Literacy and Patient Education Network (CHLPEN). She previously held policy roles with Cancer Care Ontario and the Ontario Ministry of Health and Long-Term Care. She has authored papers and presented at conferences on the topics of health literacy, strategic communications and patient and family education program building. Farrah completed her Master's degree in Health Advocacy from Sarah Lawrence College in New York.
